29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sideswith passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening: Employees engaged in production activities at the receiver hopper mezzanine were exposed to fall hazards of approximately 10 feet from the mezzanine to the ground below in that the fixed ladder providing access to the mezzanine was not equipped with a swing gate.

29 CFR 1910.147(a)(2)(ii)(B): Energy control procedures were not implemented when an employee was required to place any part of his or her body into an area on a machine or piece of equipment where work is actually performed upon the material being processed (point of operation) or where an associated danger zone exists during a machine operating cycle. On or about June 4, 2015, an employee engaged in extruder cleaning activities at the west machine feeder screw was exposed to amputation hazards in that energy control procedures were not implemented prior to accessing the point of operation.
