29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about the 14th of December 2015, employees engaged in packaging activities were exposed to amputation hazards in that the rotating table on line 2, a Rapsco Model TTP-80 Direct drive (non-variable) 20 RPM, was not guarded to protect from amputation hazards.
