Employees were using an extension cord that was missing the insulation on the energized conductor.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) For employees working at 11623 John Parrish Cove in Parrish, as observed on or about June 9, 2015. Employees were working on the roof without the use of a fall protection system. The eave to ground height measured approximately 10 feet.

Employees were using an extension ladder to access the upper working surface. The ladder had a broken rung approximately 10 feet up the ladder. The broken rung was on the inside ladder.
