29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  (a) At the Willow Walk Subdivision in Palmetto and elsewhere. Employees engaged in residential roofing construction were exposed to falls that typically range from 8'8" up to 15'. No accident prevention program was established by the employer on or about 11/01/2017.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  (a) At the Willow Walk Subdivision in Palmetto, where employees engaged in residential roofing construction using circular power saws were exposed to cuts and amputations. The employees were not trained on the safety workpractices to follow before and while using circular saws on or about 11/01/2017.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) At the Willow Walk Subdivision in Palmetto and elsewhere. Employees engaged in residential roofing construction were exposed to falls that typically range from 8'8" up to 15'. No fall protection training program was established by the employer on or about 11/01/2017.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) At the Willow Walk Subdivision in Palmetto, where employees were observed engaged in residential roofing construction. The employees had no fall protection and exposed to a fall that ranged from 15 to 88 on or about 11/01/2017.  The F. Elias Construction, Llc was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 976322, citation number 1, item number 1 and was affirmed as a final order on 06/30/2014, with respect to a workplace located in Riverview, Florida.
