29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used: a) On or about 6/10/15 at the Goodyear Tire Store Construction project located at 4972 Transit Road, Depew, New York; employees were exposed to a potential fall of approximately 20 ft. from the edge of the roof while in the process of installing a new roof. Employees were unable to demonstrate the proper selection and/or use of the fall protection systems, methods and equipment specific for the work being performed. ABATEMENT CERTIFICATION REQUIRED

WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314389065, CITATION NO. 1, ITEM NO. 3, AND WAS AFFIRMED AS A FINAL ORDER ISSUED ON 09/28/2012, WITH RESPECT TO A WORKPLACE LOCATED AT 65 GREAT ARROW AVENUE, BUFFALO, NY 14216. ABLE INDUSTRIAL SERVICES INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 967041, CITATION NO. 1, ITEM NO. 1, AND WAS AFFIRMED AS A FINAL ORDER ISSUED ON 05/27/2014, WITH RESPECT TO A WORKPLACE LOCATED AT 5897 TRANSIT ROAD, LOCKPORT, NY. 29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: a) On or about 6/10/15 at the Goodyear Tire Store Construction project located at 4972 Transit Road, Depew, New York; employees were exposed to a potential fall of approximately 20 ft. from the edge of the roof while in the process of installing a new roof. There was no fall protection system in use. NO ABATEMENT CERTIFICATION REQUIRED
