29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered.    On or about 12 June 2015, and at times prior thereto, the employer did not provide and enforce the use of personal protective equipment for the following hazards:    a) Employees using open propane flames to burn off excess plastic molding, exposing those employees to a burn hazard to the hands, arms, and body.    b) Material handlers accessed a beam to step up on to add raw materials to a Machine #23 hopper without the use of fall protection, exposing the employees to a fall hazard of approximately 6 feet 2 inches to the concrete floor below.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.    On or about 12 June 2015, and at times prior thereto, a SeaJay machine utilized to press, shear and bore out various plastic products did not have guarding or was not being maintained to prevent contact with the boring blade, exposing employees to an amputation hazard.

On or about 12 June 2015, and at times prior thereto, a 4 Speed, Wood Bandsaw was being used to cut pieces of plastic molding from product without the guard adjusted to cover all portions of the blade not being used, exposing the employees to an amputation hazard from being struck by the rotating blade.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling.    On or about 12 June 2015, and at times prior thereto, re-locatable power taps (RPTs) were being used on the Production Floor to provide power to various tools and equipment in which the RPTs were connected in series to extension cords that were plugged directly into a wall outlets, exposing employees utilizing the RPTs to an electrical fire hazard.  1) Machine #21 had an RPT plugged into an orange extension cord. The extension cord had four items plugged in at the time of the inspection, with two fans and a leak detector.  2) Machine #18 had two RPTs connected in series with power provided by an orange extension cord. At the time of the inspection, one RPT had two items plugged in, while the other had the second RPT and three other items plugged in.  3) Machine #4/1 had an RPT plugged into a yellow extension cord. The extension cord had four items plugged in at the time of the inspection.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws.  On or about 12 June 2015, and at times prior thereto, a turn table power cord had damage to the cord located near the plug end in which the internally sheathed wires were visible, exposing employees to an electrical hazard of approximately 120 volts.
