29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person designated by the employers: a) At 8001 Berry Road in Bonne Terre, MO 63628 On or about 06/09/2015, the general contractor/employer failed to initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person designated by the employer when employees were exposed to the struck-by hazard of falling shingles and roof material without wearing head protection, lacked fall protection systems when employees were exposed to elevated falls of 11 feet or greater to a concrete surface, and lacked training necessary to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) At 8001 Berry Road in Bonne Terre, MO 63628 On or about 06/09/2015, each employee working on a steep roof (5 in 12 pitch or greater) with unprotected sides and edges, approximately 11 feet above a solid concrete surface, was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19
