29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: (a) Excavation located at the intersection of Canton Street & Mayfield Road, Alpharetta, GA 30009: The employer allowed work to be performed by an employee in an excavation that was 11-feet long by 11-feet wide by 9-feet, 6-inches deep with no safe means of egress, i.e. ladder or ramp.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) in that the excavation was sloped at an angle steeper than one-half horizontal to one vertical. (a) Excavation located at the intersection of Canton Street & Mayfield Road, Alpharetta, GA 30009: Employee was exposed to soil cave-ins while working in a 11-feet deep x 11-feet long x 9-feet, 6-inches deep excavation installing a 30-inch T-Valve Assembly with no cave-in protection system in place. The trench box/shield was located outside the excavation while the employee was working inside the excavation.
