29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary. On or about September 22, 2017, at 4220 South Lee Street, Buford, Georgia: Two employees performing sewer pipe installation work were exposed to struck-by and cave-in hazards from dirt clods and rocks falling into an approximately 62-foot long, 4-foot wide, and 6.5-foot deep trench they worked in with the spoil pile set at the edge of the trench.
