29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    WORKSHOP:  The employer had not established and implemented a site specific hazardous energy control lo/to program consisting of machine specific step-by-step written procedures and employee training for the employees during servicing and maintenance of equipment, such as (but not limited to) cutters and the laser machines, machining center, press brakes, and overhead crane/under hung hoists.

WORKSHOP: The employees, as affected and other types, were not provided with training and information in the lockout/tagout program and controlling potentially hazardous and un-expected release of energy sources during servicing and maintenance of equipment, such as (but not limited to) the laser machines, machining center, press brakes, and overhead crane/under hung hoists.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.    WORKSHOP: The unused portion in the right side blade of the of the S-20 Series II, horizontal bandsaw was unguarded. 29 CFR 1910.134(k): Training and information. This paragraph requires the employer to provide effective training to employees who are required to use respirators. The training must be comprehensive, understandable, and recur annually, and more often if necessary. This paragraph also requires the employer to provide the basic information on respirators in Appendix D of this section to employees who wear respirators when not required by this section or by the employer to do so.  WORKSHOP:  The employer had not provided the basic information in the Appendix D of the respiratory standard to the employees who were optionally wearing filtering facepiece and half facepiece cartridge respirators.
