OSH ACT of 1970 Section (5)(a)(l): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were not effectively trained to safely use machines in a manner to prevent amputations.  PRESS BRAKE AREA/PRODUCTION DEPARTMENT:    On or about September 16, 2022, an employee operating a TRUMPF Trubend 7036 press brake amputated the tip of her middle finger when reaching to catch a part that had slipped from the point of operation.  Among other methods, feasible and acceptable means of abatement include:       1.	Providing employees with training in safe working procedures which include both set-up and operation of the machines.      2.	Regular evaluations conducted by supervisors to ensure that employees are operating the machines safely in accordance with company training and operating procedures.      3.	Documenting all employee training and supervisory evaluations.      4.	Anyone person(s) observed not following established safety procedures will be provided with refresher training and/or subject to disciplinary actions.

The employee was not provided with aa tool to use to place the part in position rather than putting her hand in lose proximity to the point of operation.
