29 CFR 1910.269(u)(5)(ii): Guarding of energized parts within a compartment during operation and maintenance functions to prevent accidental contact with energized parts and to prevent dropped tools or other equipment from contacting energized parts, except for fuse replacement and other necessary access by qualified persons, was not maintained. a) On or about 06/11/15 at Building 8 substation #3 at Monroe Community College; employer failed to guard the opening created when a portion of the protective barrier, which separated the fuse holder assembly for switch PS-3C and the transformer area, was removed and exposed employees, disassembling the transformer, to the energized (4.16 KV AC ) portion of this switch. ABATEMENT DOCUMENTATION REQUIRED
