29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    a) Flexcut Tool Company, Inc., metal shop, Guifil Press (PC 6-16):  On or about June 11, 2015 and at times prior thereto, the press was not equipped with any point of operation guarding.  Employees inserted parts into the press's point of operation zone by hand.  On or about May 1, 2015, an employee suffered a fingertip amputation while operating the machine.

29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent) was not completed in the detail as required by the regulation:    a) Flexcut Tool Company, Inc. -- Recrodkeeping Program --- OSHA Logs:  The entries in Column F on the OSHA 300 Log for Calendar Year 2015 were not descriptive in that the agent, object, and/or substance that directly injured or made the person ill was not noted/documented.  The entries on the log only contained a one item line noting the injury.    b) Flexcut Tool Company, Inc. -- Recrodkeeping Program --- OSHA Logs: The Column F entry for case number 4 on the Calendar Year 2015 OSHA log was not properly characterized.  The entry was characterized as a laceration.  The injury was actually a fingertip amputation.

Flexcut Tool Company, Inc. -- Recrodkeeping Program --- OSHA 301 Form: The OSHA 301 form for case number 4 on the Calendar Year 2015 OSHA 300 log was not completed properly.  Item number 16 on the OSHA 301 Form characterized the injury as a right index finger cut.   The injury was actually a fingertip amputation.
