29 CFR  1910.147(c)(4)(ii):Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  a.) Flexcut Tool Company Inc; Metal Shop-on or about 5/15/22, and times prior thereto, employees periodically performed servicing and maintenance on equipment including but not limited to the Greenerd 40- ton hydraulic press Model #HCA-401124 Serial # 05T6587. The employer's written energy control control procedures did not include specific steps for the placement and removal of lockout devices.  Additionally, the procedures did not identify 'stored energy' as an energy source.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Flexcut Tool Company Inc. on or about 6/15/22 and times prior thereto: The employer did not conduct periodic inspections for service and maintenance activities on equipment including but not limited to the Greenerd 40- ton hydraulic press Model #HCA-401124 Serial # 05T6587.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Lockout tagout procedures were not utilized during set-up operations. 29 CFR  1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a.) Flexcut Tool Company Inc; Metal Shop - on or about 6/15/22 and times prior thereto, the Greenerd 40- ton hydraulic press Model #HCA-401124 Serial # 05T6587 was not adequately guarded to prevent employees from contacting the point of operation in that employees fed parts into the machine while activating the operating cycle with a foot pedal and without any Point of Operation Guarding.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).
