29 CFR 1910.134(c)(1) as required by 1926.103:  A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    a) On or about June 15, 2015, at 616 Madison Avenue, in the alley, the employer failed to ensure that an effective respiratory protection program was in place for employees required to wear a supplied air hood respirator during abrasive blasting operations to remove paint from the outside wall of the building.  Employees were potentially exposed to lead, carbon monoxide, silica, and health hazards associated with respirator use.

No exposure assessment had been conducted.
