Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) UrbanX, Inc. on or about August 07, 2015, at the address of 520 Madison Ave. Toledo, OH: The employer failed to ensure employees were protected from falls while performing roofing activities atop a low sloped commercial structure. Employees removing old roofing material (build-up) from atop the structure were exposed to an approximate 120 ft. fall to the ground below.
