OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:    a) At the facility, on or about June 11, 2015, the employer did not furnish the employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck-by or punctured by high velocity projectiles during the rupture of polyvinyl chloride (PVC) pipe with a line pressure of 90 pounds per square inch (psi) being used to transport compressed air to operate the CNC Mill Machines, air sanders, drill motors, etc.  The piping was not approved for use with compressed air.  Fragmentation of the pipe can occur because of fatigue caused by cold contents and/or vibrations or the compressed shifting position and placing additional stress on the PVC pipe connector.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     a)  At the facility, on or about June 11, 2015, the Jet drop band saw did not have the unused portion of the blade guarded, exposing employees to an amputation hazard.

29 CFR 1910.213(d)(1): Circular handfed crosscut table saw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:     a)  At the facility, on or about June 11, 2015, an employee was using a Powermatic Table Saw without the guard for the portion of the saw blade above the table, thus exposing the employee to amputation hazards.
