29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) On or about June 15, 2015, two employees were installing flashing materials on the second story roof of a residential condo under construction up to approximately 26 feet high without fall protection, exposing employees to a fall hazard. b) On or about June 15, 2015, an employee was installing weatherproofing materials on the exterior of the residential condo under construction up to approximately 10 feet high without fall protection, exposing the employee to a fall hazard.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about June 15, 2015, two employees were utilizing 2, 24-foot Louisville extension ladders to access the first and second story roofs of the residential home under construction without extending the rails at least three feet over the upper landing surface, exposing the employees to a fall hazard up to approximately 19 feet.
