29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. On or about May 18, 2017, and at times prior thereto, the employer did not ensure metal folding machines such as, but not limited to, the Schechtl MAX 3100 had machine guards to prevent employees' hands from being caught in the machine.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: On or about May 18, 2017, and at times prior thereto, the shop industrial fan was powered by a damaged flexible cord that did not have strain relief and was showing exposed internal wiring, exposing employees to the hazard of being shocked.
