29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On June 9, 2015, the employer did not assure the rotating nip rollers associated with the laminator (Model # 15-A-54 HMRC, Serial # 6419) were adequately guarded. An employee suffered severe arm injuries while operating the machine. b) On June 19, 2015, the employer did not assure the rotating part on the laminator lower fabric roll associated with the laminator (Model # 15-A-54 HMRC, Serial # 6419) was adequately guarded. Employees assisting with pulling material from the machine at the table area were exposed to rotating parts associated with the lower fabric roll. Cool Seal USA, LLC. was previously cited for a serious violation of this occupational safety and health standard or its equivalent standard CFR 1910.212(a)(1) which was contained in OSHA inspection number 914083, citation number 1, item number 1 and was affirmed as a final order on September 19, 2013, with respect to a workplace at 232 J Street, Perrysburg, Ohio.
