29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:   (a) In the compressor room, where an eyewash was provided for employees who were changing batteries and using a jump box to boost drained batteries, access to the eyewash was partially blocked by the fan for the compressor, which was bolted to the floor, and by a closed door between the service bay area and compressor room, exposing employees to the hazards of corrosive chemical exposure to eyes and face.   (b) In the pit area, the eyewash that was provided for employees who may be exposed to particles falling from engine components while working under vehicles, including corrosive salts of battery acid, was out of service, exposing employees to the hazards of eye contact with corrosive materials, exposing employees to the hazards of corrosive chemical exposure to eyes and face.

29 CFR 1910.303(b)(1)(ii): Electrical equipment was not free from recognized hazards, based on the mechanical strength and durability, including, for parts designed to enclose and protect other equipment, the adequacy of the protection thus provided:  (a) In the compressor room, where employees were using the Siemens safety switch box for the air compressor to power it on and off daily, the box was not secured and was slipping sideways on the wall, exposing employees to the hazards of electrical shock.  (b) Outside, on the front of the building, a duplex outlet receptacle powering a 7-UP soft drink vending machine was missing the cover and the faceplate was not flush, exposing electrical components to the elements and employees to the hazards of electrical shock.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  Instance (a) In the training room, a Lasko box fan was plugged into a relocatable power tap in lieu of providing a fixed outlet, exposing employees to the hazards of fire and electrical shock.  Instance (b) In the training room, a relocatable power tap was series connected to an orange junior hard service cord in lieu of direct connection to a permanently installed receptacle and used to power heavy appliances including a small refrigerator and GE microwave oven in lieu of lighter loads such as computer peripherals, periodically causing the breaker to trip, exposing employees to the hazards of fire and electrical shock.

29 CFR 1910.305(g)(1)(iv)(D): Flexible cords and cables were attached to building surfaces:   In the training room, an orange junior hard service cord was plugged into a duplex outlet receptacle under the computer table, zip tied to flexible conduit along the wall, and suspended across overhead HVAC ductwork to the opposite side of the room, where it was powering a relocatable power tap. A GE microwave oven, small refrigerator, and cordless phone were plugged into the power tap, exposing employees to the hazards of fire and electrical shock.

In the pit area, oil mixed with water was standing on the floor, exposing employees the hazards of slips, falls, electrical shock, and chemical exposure to the skin.    The Heartland Automotive Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.22(a)(2) which was contained in OSHA inspection number 63215.015, citation number 02, item number 001, and was affirmed as a final order on 11/07/2011 with respect to a workplace located at 215 W. Rand Rd., Arlington Heights, IL 60004.
