29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous. On or about June 16, 2015, at job site location 1111 Tulane Ave, New Orleans, LA 70112, the employer allowed the employee to use electrical equipment with electrical plug missing a ground pin. Therefore there was not a permanent and continuous ground.
