29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees. At this construction site, an employee was seen egressing from a trench 3.5 feet wide and 6.9 feet deep without a safe means of egress. Employee was exposed to a cave-in hazard due to no means of egress. QRO Mex Construction Company was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 525881, citation number 1, item 1a.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) in that: At this construction site, an employee was observed egressing from a trench 3.5 feet wide and 6.9 feet deep. The employee worked inside the trench where the walls were substantially vertical without a protective system to prevent the employee. The employee was exposed to hazards associated with a cave-in. QRO Mex Construction Company was previously cited for a violation of this occupational safety and health standard contained in OSHA inspection number 525881, citation 1, Item 3.
