29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): The employer does not ensure point of operation guarding is in place to protect employees from having any part of their body in the danger zone during operation. a) This violation was observed on or about June 18, 2015 when the employer allowed employees to mute light curtains higher than needed or turn off light curtains being used as point of operation guarding. Employees were exposed to hazards associated with being caught in machinery. b) This violation was observed on or about July 15, 2015, when the Amada press brake #1203 point of operation was not guarded. Employees were exposed to hazards associated with being caught in machinery. c) This violation was observed on or about July 15, 2015, when the DiAcro press brake #0205 point of operation was not guarded. Employees were exposed to hazards associated with being caught in machinery. d) This violation was observed on or about July 15, 2015, when the DiAcro press brake #0205 foot pedal was not guarded. Employees were exposed to hazards associated with being caught in machinery. e) This violation was observed on or about July 15, 2015, when the Cincinnati press brake #0279 point of operation was not guarded. Employees were exposed to hazards associated with being caught in machinery.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: On or about July 14, 2015, the employer allowed a relocatable power tap to be plugged in to an extension cord at Amada press brake station 1203 exposing employees to hazards associated with contact with electricity. The Acme Engineering and Manufacturing Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 956971, citation number 1, item number 2 and was affirmed as a final order on May 9, 2014, with respect to a workplace located at 1820 N. York Street, Muskogee, OK 74403.
