29 CFR 1910.147(c)(7)(iii)(B): Additional retraining was not provided whenever a periodic inspection under 29 CFR 1910.147(c)(6) revealed that there were deviations from or inadequacies in the employee's knowledge or use of the energy control procedures: (a) Facility, Saw Shed, 1030 Broad Street, Greensburg, PA: On or about 6/30/2015 during installation of automatic feed roller system employees did not have keys for lockout devices on their person, as required by company lockout tag out program, instead the keys for locks used to lockout equipment were hanging inside saw shed. Equipment that was locked out was the HYD-MECH Ocean Terminator DCM 18/42 band saw.
