Employees were allowed to use fork lifts that were found to be in need of repair due to the following instances:        a. A forklift with a factory installed seat belt was found to have the seat belt damaged.       b. A forklift had a broken windshield that impaired vision while employees were lifting loads.     These conditions exposed employees to struck-by and caught between hazards.
