29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided:  Larry Tipton worksite located at 7194 Regents Park, Sylvania, Ohio: On or about June 18, 2015, the employer did not ensure an employee was wearing proper eye protection while operating pneumatic nail guns while installing shingles above the garage of a two story residential structure. The employee was exposed to potential flying object hazards.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   Larry Tipton worksite located at 7194 Regents Park, Sylvania, Ohio: On or about June 18, 2015, the employer did not ensure an employee was protected from head injuries while working below employees tearing off old shingles on a two story residential structure. Due to this condition, employees were potentially exposed to impact injuries caused from falling material and/or equipment.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501  Larry Tipton worksite located at 7194 Regents Park, Sylvania, Ohio: On or about June 18, 2015, the employer did not ensure employees were protected from fall hazards while tearing off and installing weatherproofing at the peak on the back side of the two story residential structure. Employees were exposed to a fall distance of approximately 18.00 feet from the 9/12 pitched roof.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   A. Larry Tipton worksite located at 7194 Regents Park, Sylvania, Ohio: On or about June 18, 2015, the employer did not ensure the side rails of the extension ladders being used on the front side of the two story residential structure were extended at least three feet above the landing surface. Five employees were exposed to falls while using the ladders to access the roof.
