29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: (a) On or about June 18, 2015 at the above addressed worksite, employees of the controlling contractor and its subcontractors had to walk by unguarded floor openings measuring 45 inches by 171 inches to access their respective work areas, exposing the employees to the hazards of falling up to 10 feet to the lower floor.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stair rail system along each unprotected side or edge: (a) On or about June 18, 2015 at the above addressed worksite, employees of the controlling contractor and its subcontractors used a stairway with 15 risers and no stairwell to access the second story of a multi-unit condo building, exposing the employees to the hazards of falls up to 10 feet to the lower floor.
