29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. a. At the workplace, in the metal processing department, the operation of cutting 053 brackets using a band saw required the operator's hands to be within 4 to 6 inches of the cutting blade while securing the bracket patterns by hand, thereby exposing employees to an amputation hazard. On or about June 11, 2015, an employee suffered an amputation while performing the operation of cutting 053 brackets using a band saw.
