29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about June 18, 2015, at 2440 Whitehead Place, Suwanee, Georgia:  An employee performing roofing work was exposed to struck-by hazards from falling roofing shingles, roofing felt, and tools of the employees working above her when she worked beneath those employees without using head protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about June 18, 2015, at 2440 Whitehead Place, Suwanee, Georgia:  a) Two employees performing roofing work were exposed to a fall hazard of approximately 18.3-feet and greater when they used an aluminum extension ladder that was not extended three feet above the roof landing.   b) An employee performing roofing work was exposed to a fall hazard of approximately 8.9-feet and greater when she used an aluminum extension ladder that was not extended three feet above the porch roof landing.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about June 18, 2015, at 2440 Whitehead Place, Suwanee, Georgia:  a) Two employees performing roofing work on an approximately 10/12 slope roof were exposed to a fall hazard of approximately 18.3-feet and greater when they worked without using fall protection.   b) An employee performing roofing work on an approximately 4/12 slope porch roof was exposed to a fall hazard of approximately 8.9-feet and greater when she did not use fall protection.
