29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:     On or about June 19, 2015, during the installation of roofing material at a job site located at 1822 Marietta Ave, Parma Ohio 44134; the employer did not ensure employees working on a residential roof were protected from a fall of approximately 8.8 to 10.11 feet by utilizing a fall protection system by providing fall protection training or fall hazard recognition.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):      On or about June 19, 2015, during the installation of roofing material at a job site located at 1822 Marietta Ave, Parma Ohio 44134; the employer did not ensure employees working on a residential roof were protected from a fall of varying height of approximately 8.8 to 10.11 feet by utilizing a fall protection system.     (a)  The Sunset Construction CO. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 990796, citation number 1, item number 1 and was affirmed as a final order on 11/26/2014, with respect to a workplace located at 6998 Pearl Rd, Middleburg Heights Ohio 44130.    (b) The Sunset Construction CO. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 991654, citation number 1, item number 1 and was affirmed as a final order on 11/26/2014, with respect to a workplace located at 600 Eastwood Rd, Hinckley Ohio 44233.
