29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Location: 22 Fieldstone Way, Plymouth, MA.  Employees were exposed to fall hazards at 10 to 26 feet while re-roofing a residential home and shed, where no fall protection was provided or used.   a) Employees were exposed to fall hazards on the roof of the home.  b) Employees were exposed to fall hazards on the porch roof.  c) Employees were exposed to fall hazards on the shed roof.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Location: 23 Fieldstone Way, Plymouth, MA.  Employees were exposed to falls greater than 12 feet from the shed roof, and greater than 16 feet from the garage roof, while re-roofing the shed and house, where the ladders were not 3 feet above the landing.  a) The first ladder was resting on the side of the shed, approximately 1 foot over the roof edge.  b) The second ladder was resting at the peak of the garage roof, approximately 1 foot over the roof edge.
