29 CFR 1910.178(p)(1): Powered industrial truck was found to be in need of repair, defective, or in any way unsafe, the truck and was not taken out of service until it was restored to safe operating condition: On or about June 22, 2015, and at times thereto, employees were allowed to operate powered industrial trucks that were in need of repair and not taken out of service until it was restored to safe operating condition. This condition exposed employees to fire, struck-by and caught-between hazards. a) The Yale Forklift's backup alarm and/or strobe light was not operational and the fire extinguisher missing. b) The Cat-246D Skid Steer Loader tires were worn-out.
