29 CFR 1910.178(a)(5): If the truck is equipped with front-end attachments other than factory installed attachments, the user shall request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered.    WORKSHOP:  The Yale forklift truck, which was equipped with a front-end attachment to handle wooden pallets and equipment was not equipped with an identification tag or an equivalent indicating the load capacity rating and other information. WORKSHOP:  The forged alloy steel sling chains, used to hoist laundry equipment with the Yale powered industrial truck (PIT) did not have permanently affixed tags and/or identifications stating the size, grade, rated capacity, and other information. 29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met.    WORKSHOP: The hazard communication program was generic and was not written site specific where the employees were required required to work with chemicals, such as (but not limited to) Safety Kleen and WD-40.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety data sheets.  WORKSHOP: The employees, required to work with chemicals, such as (but not limited to) Safety Kleen and WD-40 were not provided with training and information on the chemical hazard communications.
