29 CFR 1926.20(b)(2): The employer failed to initiate and maintain programs that provided for frequent and regular inspections of the job sites, materials, and equipment by competent persons designated by the employer:    At the project, frequent and regular inspections of the job site, material and equipment were not made.  The Vice President of the employer said he was onsite that morning to drop off supplies, but he did not inspect the site to ensure it complied with OSHA regulations prior to leaving.  Employees were exposed to the hazards of contacting electricity and falling to the same level.

29 CFR 1926.34(c): Means of egress was not continually maintained free of all obstructions or impediments to full instant use in the case of fire or other emergency:    Near the east side door from the interior to the exterior, the means of egress was not continually maintained free of obstructions and impediments as there were a compressor, electrical cords, air hoses, pieces of lumber, and plastic bags on the floor in the aisle to the door.  Employees were exposed to the hazard of fire and falling to the same level.

29 CFR 1926.404(b)(1)(i): The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.  These requirements are in addition to any other requirements for equipment grounding conductors:    Near the east side exterior of the project, there were no ground-fault circuit-interrupters or an assured equipment grounding conductor program to protect employees that used a 110 VAC DeWalt miter saw to cut trim boards.  Power was supplied to the saw through a flexible cord plugged into a 110 VAC wall outlet.  The outlet was not a GFCI outlet.  Employees were exposed to the hazard of contacting electricity.

29 CFR 1926.405(b)(2): In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy:    Inside the building on the first level, there was no cover on the 110 VAC outlet that supplied power through a flexible cord to a DeWalt miter saw used to cut trim boards.  The bottom of the 2 inch wide by 3.25 inch high outlet box was located 12 inches above the floor.  Employees were exposed to the hazards of contacting electricity.
