29 CFR 1904.29(b)(1): What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.  a.  At the time of inspection, the employer had not recorded information on the OSHA 300 Log of Work-Related Injuries and Illnesses in column (F) correctly for calendar year 2015.  On or about April 12, an employee had an injury to  the wrist, but there was no description of what caused the injury, which specific (left or right) wrist was injured, or what the employee was doing when the injury occurred.   b. At the time of inspection, the employer had not recorded information on the OSHA 300 Log of Work-Related Injuries and Illnesses in column (F) correctly for calendar year 2015.  On or about  May 11, an employee had an injury to  the forearm but there was no description of what caused the injury, which specific (right or left) forearm was injured, or what the employee was doing when the injury occurred.   c.  At the time of inspection, the employer had not recorded information on the OSHA 300 Log of Work-Related Injuries and Illnesses in column (F) correctly for calendar year 2015.  On or about June 2, an employee had an injury to  the Achilles but there was no description of what caused the injury, which specific (left or right) Achilles was injured, or what the employee was doing when the injury occurred.    Abatement certification required within ten days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
