29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions. An inspection shall be conducted by the competent person prior to the start of work and as needed throughout the shift: a) Jobsite, 2501 Smallman Street, Pittsburgh, PA: On or about June 15, 2015, an employee was exposed to cave-in hazards while working in an unprotected excavation and there was not a competent person onsite to perform daily inspections of the excavation b) Jobsite, 2501 Smallman Street, Pittsburgh, PA: On or about June 19, 2015, an employee was exposed to cave-in hazards while working in an unprotected excavation and there was not a competent person onsite to perform daily inspections of the excavation.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c): a) Jobsite, 2501 Smallman Street, Pittsburgh, PA: On or about June 15, 2015, an employee was working in an unprotected excavation to clear the dirt around the new water line for the Plumbers to access, and the excavation was approximately 7-8 feet deep at the deepest point next to the building. b) Jobsite, 2501 Smallman Street, Pittsburgh, PA: On or about June 19, 2015, an employee was working in an unprotected excavation to tap into the main sanitary line in the street and the excavation was approximately 8.5 feet deep.
