29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels are not protected by guardrail systems, safety net system, or personal fall arrest system, nor are employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):           Employees are exposed to fall hazards while engaged in residential construction activities 6 feet or more above lower levels.  The employer does not protect each employee by means of a guardrail system, safety net system, personal fall arrest system, or an alternative fall protection measure.       The following instances most recently occurred on June 23, 2015, at the above addressed jobsite:          a) Employees engaged in residential construction activities are not protected from falling from an unprotected sides, thereby exposing employees to a 12.9 foot fall hazard from the second floor to grade.       b) Employees engaged in residential construction activities are not protected from falling through floor holes, thereby exposing employees to falls up to 13.1 feet.         c)  Employees engaged in residential construction activities are not protected from falling through an unguarded window, thereby exposing employees to a 14.6 foot fall hazard from the second floor to grade.                To abate this hazard in the future, the employer must ensure that its employees performing residential construction activities at six feet or greater are protected from falls, and that no employee is working without adequate fall protection.     Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.     On June 23, 2015, employees were exposed to an electrical shock hazard when using extension cords plugged in to a portable generator. A ground fault circuit interrupter was not used and the employer did not institute an assured equipment grounding conductor program, as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section.     Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

On June 23, 2015, employees were using an extension cord without a grounding pin, thereby exposing employees to an electrical shock. 29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail:     On June 23, 2015, employees were using a stairway not equipped with a handrail to access the second floor working area, thereby exposing employees to fall hazards of approximately 10 feet.  The employer did not ensure stairways having four or more risers were equipped with at least one handrail.
