29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights is no more than 1 inch (2.5cm) wide: At this worksite, employees worked from the top tier of a scaffold and the working/walking platform was not fully decked exposing employees to a fall over ten feet to the ground below.

29 CFR 1926.451(g)(4)(xv): Crossbracing used in place of a midrail was unacceptable because the crossing point of two braces was not between 20 inches (0.5 m) and 30 inches ((0.8 M) above the work platform or as a toprail crossing point of two braces were not between 38 inches (0.97 m) and 48 inches (1.3 m) above the work platform. The endpoints at each upright were more than 48 inches (1.3 m) apart: At this worksite, employees applied stucco to the exterior of a building from a scaffold platform which was over ten feet above ground, where the crossbracing was used in place of a midrail and the crossing point was not between twenty and thirty inches above the platform.
