The employer is failing to ensure that all employees are protected from fall hazards when entering and exiting the landing surface of a residential roof. Employees were using a twenty-eight foot extension ladders to enter and exit the roof. This ladder did not have the side rails extended to at least three feet above the landing surface on the edge of the level of the roof. The ladder was extended one feet above the eave. The ground to eave height of this house measured approximately 11 feet.
