Detachable side protectors:  The employer is failing to ensure that all employees are protected from hazards involving injury to the eyes when operating pneumatic nail gun. This event was most recently documented at a jobsite located at or about 17011 Rachel Snowden Parkway, Bennington, Nebraska, 68007 on March 21, 2019. An employee was working on the roof of a new residential structure, operating a pneumatic nail gun without the use of an appropriate eye protection.

The employer is failing to ensure that all employees are protected from fall hazards when using fork-lifts with scaffold platforms that are not securely attached to the fork. This event most recently documented at the jobsite located at 17011 Rachel Snowden Parkway, Bennington, NE 68007 on March 21, 2019. Employees were exposed to falls of over 11 feet to the ground below.

This event was most recently documented at the jobsite located at 17011 Rachel Snowden Parkway, Bennington, Nebraska, 68007 on March 21, 2019. Employees were working on the roof of a new residential structure without utilizing fall protection and were exposed to falls of over 11 feet to the ground below. The pitch of the roof was 6:12.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  The employer is failing to ensure that all employees are protected from fall hazards when entering and exiting the landing surface of a residential roof. This event was most recently documented at the jobsite located at 17011 Rachel Snowden Parkway, Bennington, Nebraska 68007 on March 21, 2019. Employees were using a twenty-eight foot extension ladders to enter and exit the roof. This ladder did not have the side rails extended to at least three feet above the landing surface on the edge of the level of the roof. The ladder was extended one feet above the eave. The ground to eave height of this house measured approximately 11 feet.

The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards.  The employer failed to protect the employees from fall hazards while utilizing ladders by providing ladder training. This was most recently documented at the jobsite located at 17011 Rachel Snowden Parkway, Bennington, Nebraska 68007 on March 21, 2019. The employer is failing to train employees to recognized fall hazards associated with the proper use and placement of ladders.
