29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    On or about June 24, 2015, at the worksite located at 5001 Ridge Rd, Wadsworth, OH 4428, the employer did not ensure employees were protected from a fall hazard of approximately 11 feet while tearing off asphalt shingles on a 4/12 pitch commercial roof.  No safety nets, guardrail systems, PFAS, or safety monitor was be utilized at the time of the inspection.    The Lindsey Construction & Design Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 583739, citation number 01, item number 01 and was affirmed as a final order on 12/09/2009, with respect to a workplace located at 227 Ferguson Ave., Tallmadge Ohio.    The Lindsey Construction & Design Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 313671943, citation number 02, item number 01 and was affirmed as a final order on 9/20/2012, with respect to a workplace located at 20296 Emery Rd., Cleveland Ohio, 44128.
