On or about June 24, 2015, during the installation of a residential home framing and assembly project located at 514 Ashwood Lane, Painesville OH, the employer did not conduct frequent and regular inspections of the job site where employees were exposed to fall hazards and falling/flying debris from working surfaces of heights approximately 12 feet.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about June 24, 2015, during the installation of a residential home framing and assembly project located at 514 Ashwood Lane, Painesville OH, the employer did not ensure that employees (4 total including employer and a foreman) were wearing protective eyewear while working with powered nail guns.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   On or about June 24, 2015, during the installation of a residential home framing and assembly project located at 514 Ashwood Lane, Painesville OH, the employer did not ensure that 3 employees were protected from a fall hazard of approximately 12 feet by utilizing a personal fall arrest system (pfas) or having a fall protection device in place.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support).  On or about June 24, 2015, during the installation of a residential home framing and assembly project located at 514 Ashwood Lane, Painesville OH, the employer did not ensure that an employee set up an aluminum extension ladder appropriately which was being used at the side of a residential home which was under construction.  The ladder height was approximately 10-12 feet and the width spread at the time of inspection was greater than 3 feet.
