29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: On June 23,2015, on the exterior east side of a commercial building under construction, a forty foot aerial lift was used by employees without wearing a body belt and attaching to the boom or basket. Employees were exposed to fall hazards sixteen feet above the ground level.
