29 CFR 1926.302(b)(2): Safety clips or retainers were not securely installed or maintained on pneumatic impact (percussion) tools to prevent attachments from being accidentally expelled: On or about June 24, 2015 and in times prior thereto the hose connections for a pneumatic hammer set used to secure tilt up concrete walls did not have safety clips. The hose was located on the construction site exposing employees to the hazards of being struck by whipping hose.
