29 CFR 1926.451(a)(6): Scaffolds were not constructed in accordance with the design by a qualified person: Southwest side: On or about May 31, 2017, employee without fall protection were exposed to fall hazards greater than 6 feet to the ground below, while welding tube steel to columns on the steel from a makeshift/wooden pallet scaffold platform that was elevated with a JLG Telehandler forks.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform is not attached to the fork or the fork-lift is moved horizontally while the platform is occupied: Southwest side: On or about April 27, 2017, employees were exposed to a fall hazards, when using a temporary elevated work platform supported by a JLG Telehandler (Model: G10-55A), which was not securely attached to the forks.
