29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end, and projected more than one half the diameter of the shaft:  (a)  H-1 Garnet Line:  Employees were exposed to a grooved horizontal rotating shaft end that was 1 inch in diameter and projected 2 inches from the H-1 Garnet Machine.  The shaft end was 15 inches above floor level and was not protected with a shaft end guard, as observed on August 27, 2015.  (b)  H-1 Garnet Line:  Employees were exposed to a grooved horizontal rotating shaft end that was 1 inch in diameter and projected 3 inches from the H-1 Garnet Machine.  The shaft end was 27 inches above floor level and was not protected with a shaft end guard, as observed on August 27, 2015.  (c)  H-1 Garnet Line:  Employees were exposed to a square horizontal rotating shaft end that was 2 inch in diameter and projected 7 inches from the H-1 Garnet Machine.  The shaft end was 24 inches above floor level and was not protected with a shaft end guard, as observed on August 27, 2015.  Abatement Certification Required for this Item.

29 CFR 1910.242(b):  Compressed air used for cleaning purposes where not reduced to less than 30 p.s.i.:  (a)  Re-work/Re-sort Area:   An employee used compressed air to clean off and blow off fibers and debris from re-worked product that was not reduced to less than 30 p.s.i.  The compressed air was measured at 91 p.s.i., as observed August 27, 2015.

29 CFR 1910.1200(e)(1):   The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    (a) Troy LLC:  Employees operate the H-1 Garnet machine that uses fiberglass to produce synthetic fabrics.  The employer has not developed or implemented a written hazard communication program for employees exposure to the fiberglass, as determined on July 24, 2015.      ABATEMENT NOTE:  A written program shall include a description of how the criteria for the following will be met:  1.        Labeling and other forms of warnings;  2.        Safety Data Sheets;  3.        Employee information and training.      Additionally, a list of hazardous chemicals known to be present in the workplace must be compiled.  Methods used to inform employees of the hazards associated with non-routine tasks and the informing of contractors of workplace hazards, including a description of the labeling system used in the facility and the availability of safety data sheets must also be addressed.  The written program must be made available upon request.   Abatement Certification Required for this Item.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a) Troy LLC:  Employees operate the H-1 Garnet machine that uses fiberglass to produce synthetic fabrics.  The employer has not provided employees with information and training on the hazards of the fiberglass once it was introduced into the workplace, as determined on April 22, 2015.   Abatement Certification Required for this Item.
