29 CFR 1910.179(j)(2)(iii): Monthly inspections of hooks, with a certification record which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the hook inspected, were not performed:   1) Fabrication Shop - The employer had not conducted annual or monthly crane inspections on the 3-ton Detroit Hoist, 5-tons Robin & Meyers and 10-ton Shaw cranes that employees used on a daily basis to hoist material.  The 10-ton crane Shaw serial number 2623 had a damaged clip.  Employees were exposed to a struck-by hazard.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:   Fabrication Shop in Restroom - The electrical outlet box was not completely enclosed with covers designed by the manufacturer.  The electrical outlet was missing a faceplate and supplying power to the heat tank that keeps the pipes from freezing.
