The most recent occurrence of this was found during the inspection of the facility located at 540 South Street, Lincoln, NE where the employer had an employee reach into the inadequately guarded gear assembly on the Hesser bagging machine to clean the glue spillage while the machine was still operating resulting in the employees glove and hand being pulled into the gears and injuring the employee.
