29 CFR 1926.451(c)(2)(ii): Unstable objects shall not be used to support scaffolds or platform units: On the second level above the garage, unstable objects were used to support a scaffold platform, as three CMUs were stacked on their ends at each of the four corners of the scaffold platform to support the platform. Employees handling rebar on the scaffold platform were exposed to the hazard of falling from an elevation of about 46.5 inches to the concrete surface below.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure: (a) On the second level above the garage, there was no fall protection for employees that were working and walking. Employees were exposed to the hazard of falling from an elevation of about 12 feet to the compacted gravel below. (b) On the second level above the stairs from the first to second level, there was no fall protection for employees working and walking. Rebars that protruded vertically from the CMUs were spaced about 16 inches apart and did not provide fall protection. Employees were exposed to the hazard of falling onto a concrete stairway platform about 9 feet below and onto stairs thar varied from about 9 to 14.5 feet below.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail: On the stairway from the first level to a stairway platform, there was no handrail when there were 10 risers. Each riser was about 6.5 inches high, and the stairs were about 4 feet wide. Employees that used the stairs to access the second level were exposed to the hazard of falling down the stairs.
