29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(c)(4):    a) Laundry Room - The employer failed to develop and utilize a lockout procedure for employees performing duties involving maintenance on a roller press. On or about June 25, 2015 the electrical disconnect on the Challenger press  was not locked out prior to an employee attempting to grease and perform other maintenance on the unit.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:     a) Laundry Room: The employer failed to ensure that an eyewash/shower unit was available for employees immediate use in the event of emergency. Employees were exposed to materials such as, but not limited to, sodium hypochlorite, liquid alkali, Ultraterge Lin and other injurious materials while performing washing of linens and other articles.

29 CFR 1910.264(d)(2)(i)(a): No safeguard, safety appliance, or device attached to, or forming an integral part of any machinery shall be removed or made ineffective except for the purpose of making immediate repairs or adjustments. Any such safeguard, safety appliance, or device removed or made ineffective during the repair or adjustment of such machinery shall be replaced immediately upon the completion of such repairs or adjustments.      a) Laundry Area - The employer failed to ensure that the Challenger press operating components were guarded during the equipment operations. On or about June 25, 2015, employees were operating the Challenger press while the pulley belts and chain and sprocket were exposing employees to being caught in between moving parts.

Employees were handling linen saturated with blood and other bodily fluids from the embalming process.

29 CFR 1910.1048(n)(2):  Employers shall provide such information and training to employees at the time of initial assignment, and whenever a new exposure to formaldehyde is introduced into the work area. The training shall be repeated at least annually.      a) Angel Linen Services: The employer failed to ensure that all employees exposed to the hazards of formaldehyde received annual training. Employees were engaged in performing activities involving the pick up  and cleaning of contaminated linen from funeral homes.
